In-Network vs. Out-of-Network Fees

CAN I USE MY HEALTH INSURANCE?

YES! You absolutely can use your health insurance to help pay for mental health treatment. At Empathy, we just don’t bill the insurance company on your behalf. So instead of in-network coverage, we would be considered out-of-network coverage.

HOW DOES THIS WORK?

We provide you automatically with superbills (medically coded receipts) that allow YOU to submit for reimbursement from the same health insurance company who takes your premium every month.

WHAT DOES THAT MEAN?

Almost every health insurance company has in-network coverage (that’s where you pay the $20 copay) and out-of-network coverage (that’s where they pay between 50%-80% for medical services you self-select that are not on their in-network list).

WHAT DOES THAT MEAN FOR MY POCKET?

Example of Superbill Reimbursement Ranges:

·                Visit Cost: $175

·                Insurance Reimbursement (50%-80%): $87.50 - $140

·                Your Actual Cost After Reimbursement: $35 - $87.50

Our Fee Structure

Our practice does not currently accept insurance and is self-pay only.

Health insurance mandates tend to limit time allotted to each patient visit and simply do not match our patient-centered model of care.

We understand affordability is a key component to access and we are happy to provide a “superbill” for our patients to submit for out-of-network reimbursements from their insurance companies (or HSA/FSA plans). Additionally, our providers only schedule follow-up appointments with patients when they are actually required for effective treatment vs. requiring a high frequency of appointments for stable patients to collect more copays.

Payments are due at the time of service.

We accept most major credit cards and Health Savings and/or Flexible Spending Account cards.

  • 60-Minute Evaluation Visit for New Patients - $250

  • 30-Minute Follow Up Visit (Combined medication management/therapy) - $125

  • Depending on complexity: $25-$100

    If needed, your provider will provide letters or complete medical forms for a reasonable fee. Please allow 1 business week for your paperwork or letter to be completed. Complex paperwork may require an appointment.

    Note: for FMLA/Disability forms we require the patient to have been established at our practice for 6 months and seeing your prescribing provider every 4-6 weeks.

  • New Patient Visit - $200

    Follow Up Visit - $100

    All appointments must be cancelled at least 48 hours in advance of the appointment time or the patient will be charged the above missed appointment fee. If a patient arrives more than 10 minutes past the scheduled appointment time, it may be necessary to reschedule the appointment and a missed appointment fee may be charged.
